Coordinated Care Program FAQs
The Coordinated Care program allows you and your covered dependents to fill prescriptions for diabetes, high blood pressure and high cholesterol for $0 as long as you see participating Hackensack Meridian Health-owned providers; follow/adhere to the prescribed care regiment and fill your eligible prescriptions at our in-house pharmacies. This program is voluntary and available to covered team members and their dependents. Through the designated care coordinators, you’ll also have access to a team of caregivers that can provide you with the knowledge, tools and self-care techniques that will help you improve your health and deal with these chronic health conditions.
Benefit eligible team members and their dependents who are enrolled in any of Hackensack Meridian Health medical plans are eligible to participate in the Coordinated Care Program.

Designated Care Coordinators are assigned and can assist in providing you with the knowledge, tools and self-care techniques that will help you improve your health and deal with chronic health conditions.
The plan is effective as of 1/1/19. If you have met the criteria, by speaking with the designated care coordinator, and Hackensack Meridian Health owned provider, and had your prescription filled at an on-site pharmacy, you may fill this form out for reimbursement.
If the drug is on the OptumRx formulary, it is covered. You may access the formulary by clicking here. Just like the regular prescription plan, if your provider writes a prescription for a brand name drug and there is a generic equivalent available, you must take the generic version or pay full price for the brand name script.
And, just like the regular prescription plan, the cost will be waived if you have successfully gone through the prior authorization process. Covered drugs include generic drugs and brand name drugs on the OptumRx Formulary drug list for which there is no generic alternative.
If you are enrolled in the Basic High Deductible Plan, you would have to meet your designated deductibles prior to the plan paying at a $0 Co-Pay.
All HMH in-house pharmacies are participating. HMH in-house pharmacies can be found by clicking here. More in-house pharmacies are coming in 2019 at JFK, Raritan Bay and Palisades.
We will attempt to include in-house pharmacy access to as many non-hospital worksites as possible via delivery couriers. We’ll provide more details about this option in the coming months.
No – you must use an HMH in-house pharmacy.
Yes – diabetic supplies such as one-touch strips and lancets are covered under the program, if you meet the criteria for the plan. Insulin Pump Supplies are available through Hackensack Meridian at Home. However, if you are enrolled in the Basic High Deductible Plan, you would have to meet your designated deductibles prior to the plan paying at a $0 Co-Pay.
Take Advantage of Our Wellbeing Incentive Program
Hackensack Meridian Health launched the 2019 Wellbeing Incentive Program in partnership with Bravo (our wellbeing vendor) to help team members learn more about their health. The program is open to all benefit-eligible team members and their spouses enrolled in the medical plan and is completely voluntary.
You and your spouse can earn a $250 incentive reward for completing the three program requirements:
- An online health risk assessment
- A biometric screening either conducted on-site or at a participating LabCorp location
- An annual physical by HMH partner or a network provider
Here are the steps you need to take to participate in the Wellbeing Incentive Program and earn your $250 incentive reward:
Step 1: Register from now through Thursday, October 31 at bravowell.com/hmh and create your account. You need to complete the registration process in order to access your results and incentive amount through your web account.
Step 2: Complete a Health Risk Assessment (HRA) at bravowell.com/hmh by Thursday, October 31. Once completed, you will receive two reports: a personal report that describes the impact of each risk factor on your health including a wellness score, and a physician summary report that can be easily printed and shared with your provider to initiate a conversation.
Step 3: Complete a biometric screening either on-site or at a participating LabCorp location by Thursday, October 31. Here are the screening options:
a. Extended on-site screenings will be available in the northern, central and southern regions, (two per region), on the following dates: Thursday, October 17, Friday, October 18, Monday, October 21 and Tuesday, October 22. Follow the step-by-step instructions at bravowell.com/hmh to schedule a screening appointment at a location that’s convenient for you.
b. Visit a participating LabCorp Location by Thursday, October 31. Download a LabCorp voucher from your participant portal and schedule an appointment at a LabCorp location near you. There is no need to submit your results to Bravo. Please note: any additional testing requests outside of the pre-approved panel will be at the expense of the team member.
Step 4: Complete an Annual Physical from November 1, 2018 through Thursday, October 31. Schedule an annual screening appointment with your health care provider. Or, if you already had a wellness visit with a participating provider after November 1, 2018, you are set! Your information will be provided by Horizon. This annual preventive screening visit is covered at 100% when you utilize a Hackensack Meridian Health or in-network provider. Remind your provider that the screening should be coded as an “Annual Physical” so your insurance will cover it at 100%. Also please note a second health screening performed in the same calendar year may not be covered at 100%.
Participants do not need to meet established wellness goals in order to receive the Wellness Incentive reward; participation is based solely on completing the three requirements.
Program participants who complete the three program requirements will start to receive their incentives in early October.

Wellbeing Incentive FAQs
You are required to complete three steps in order to earn the 2019 Wellbeing Incentive.
1. Complete biometric screening, 2. Complete the health risk assessment (HRA), 3. Get an annual physical exam.
The program is voluntary and is open to all benefit-eligible team members and spouses. By participating and completing the three program requirements, team members and spouses who are benefits eligible and enrolled in the medical plan can each earn a $250 incentive. There are no wellness goal requirements to meet.
Visit www.bravowell.com/hmh and follow the step-by-step instructions to create an account. Complete the registration step by providing Bravo with your required information. Your results and incentive amount will be posted to your web account when available for viewing.
All accounts have been reset. Team members and spouses will be prompted to create a new account during registration.
A biometric screening is a series of tests to assess your risk for chronic health conditions that will provide you the opportunity to better manage your health. These tests are administered by a venipuncture blood draw.
Biometric screening appointments will be offered starting Thursday, June 20. Here are the screening options:
- Attend your on-site screening from Thursday, July 18 through Thursday, October 31, 2019.
- Visit a participating LabCorp location between Thursday, July 18 and Thursday, October 31, 2019.
Yes! We encourage all participants to schedule an appointment for their biometric screening. Details to schedule an appointment can be found in the program guide or listed below.
You should expect the appointment to last for approximately 15 minutes.
Yes. You are encouraged to fast for the most accurate screening results. Fast 10-12 hours prior to your scheduled screening time and drink plenty of water. Take nothing by mouth except water and medications. If you are diabetic or hypoglycemic, please follow your doctor’s instructions regarding fasting.
The screenings are conducted by Wellness Corporate Solutions, a partner of Bravo Wellness. Both are reputable national organizations.
Absolutely. Only you, Bravo and Wellness Corporate Solutions have access to the results. Personal Health Information (PHI) is not shared with Hackensack Meridian Health. Hackensack Meridian Health strictly adheres to the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and Protected Health Information (PHI) protocols.
Schedule an on-site screening beginning on Thursday, June 20. Follow the step-by-step instructions at http://www.bravowell.com/hmh to schedule a screening appointment at your work location. Screening dates for each location will be posted on your portal.
If you are unable to schedule an on-site appointment, you may have your biometric screening completed off-site at a participating LabCorp. Download a LabCorp voucher from your Bravo portal and schedule an appointment at a LabCorp location near you. Off-site biometrics will be offered starting Thursday, June 20 for the 2019 Wellbeing Initiative.
No, there is no need to submit your results to Bravo.
IMPORTANT - Any additional testing requests outside of the pre-approved panel will be at the expense of the participant.
Knowing your numbers is the key to your health. The data from your biometric screening provides you with valuable information about your health status. You may wish to discuss your results with your primary care physician.
Beginning Thursday, June 20 visit http://www.bravowell.com/hmh and follow the step by step instructions to create an account.
You are required to complete the HRA on Bravo’s website by visiting www.bravowell.com/hmh. Once you create an account and log in, complete the following two steps:
- Step 1. Register
- Step 2. HRA completion
Once completed, you will receive two reports: a personal report that describes the impact of each risk factor on your health including a wellness score, and a physician summary report that can be easily printed and shared with your provider to initiate a conversation. These reports will be available online for you to download/access. You will receive an email notification stating that your reports have been posted. No paper copies will be sent.
Schedule an annual physical with a participating provider, (HMH Partner or Horizon network provider). If you have previously completed your annual physical appointment after November 1, 2018, with an in network HMH Partner or Horizon provider, you are set! Your information will be provided by Horizon. This annual physical visit is covered at 100% when you utilize an in-network provider.
IMPORTANT - Remind your provider that the appointment should be coded as an “Annual Physical” so your insurance will cover it at 100%.
No, biometric screening results cannot be submitted by your doctor.
Bravo will send email based notifications to team members and spouses with updates for completion status. Bravo will receive Annual Physical completion detail from Horizon on a bi-weekly basis. If you have already completed your Annual Physical, you will see this appear on your dashboard once received by Bravo.
Each team member, spouse/domestic partner/civil union participant should create their own online accounts at www.bravowell.com/hmh. They can log in at any time to review their completion status.
If you believe your completion status is incorrect, please contact the Bravo Wellness Call Center at 1-833-786-9492 or email@example.com.
In order to receive the incentive of $250, all three steps must be completed by October 31, 2019 and will be paid out shortly after our system registers that you have completed all three steps.
No. You cannot complete the three steps after the initial deadline. One lump sum incentive will be paid out to participating team members and spouses who are enrolled in the medical plan and complete the required steps.
If all three steps are completed by the requested deadlines and confirmation has been received from the vendors, your incentive will be paid out shortly thereafter our system registers that you have completed all three steps.

Take Advantage of Your Fitness Center Discount
Did you know that as an HMH team member you have access to a discounted fitness membership? As part of Hackensack Meridian Health’s continued harmonization, all full and part time team members are eligible to receive $20 off their monthly membership at a Hackensack Meridian Health partner’s fitness center. To take advantage of the discount and as part of our Wellness initiative, team members will need to do the following:
- Going to the fitness center at least 8 times per month
- Visiting with a health coach or trainer free of charge at the fitness center once every 3 months.
You will be reimbursed with the discount on a quarterly basis through your paycheck. For those team members who have met the requirements for the first quarter, you will receive your discount during the month of May.

Fitness Discount FAQs
No, this program will replace all other fitness and wellness programs offered by Hackensack Meridian Health.
All full time and part time team members can take advantage of the discount, including team members at Carrier Clinic.
No, the contract will be month-to-month.
If you are unable to meet the 8 visit requirement in a month, then you will not be reimbursed for that month’s discount.
You will be reimbursed on a quarterly basis. You will receive your first reimbursement (for the months in the first quarter of 2019) in one of your April 2019 paychecks.
