Wellbeing Incentive Program

To help team members learn more about their health, Hackensack Meridian Health has launched the 2019 Wellbeing Incentive Program in partnership with our wellbeing vendor, Bravo. See below for a quick overview, program guide and FAQs:

  • The program is open to all benefit-eligible team members and their spouses covered by the medical plan and is completely voluntary.
  • Participating team members and spouses can earn a $250 incentive reward for completing the three program requirements – there are no wellbeing goal requirements to meet.
  • Registration is open from now through Friday, September 20 at WWW.BRAVOWELL.COM/HMH.
  • Participants must complete an online Health Risk Assessment (HRA) at WWW.BRAVOWELL.COM/HMH between now and Friday, September 20.
  • Participants must complete a biometric screening either on-site or at a LabCorp location from Thursday, July 18 through September. The on-site screening appointment schedule is available at WWW.BRAVOWELL.COM/HMH.
  • Participants must complete an annual physical with their health care provider from November 1, 2018 through Friday, September 20.

Wellbeing Incentive FAQs


What are the requirements for the 2019 Wellbeing Incentive Program?

You are required to complete three steps in order to earn the 2019 Wellbeing Incentive.
1.  Complete biometric screening, 2. Complete the health risk assessment (HRA), 3. Get an annual physical exam.

Who can participate?

The program is voluntary and is open to all benefit-eligible team members and spouses. By participating and completing the three program requirements, team members and spouses who are benefits eligible and enrolled in the medical plan can each earn a $250 incentive. There are no wellness goal requirements to meet.

How do I get started?

Visit www.bravowell.com/hmh and follow the step-by-step instructions to create an account. Complete the registration step by providing Bravo with your required information. Your results and incentive amount will be posted to your web account when available for viewing.

What if I had a Bravo account last year?

All accounts have been reset. Team members and spouses will be prompted to create a new account during registration.

What is a biometric screening?

A biometric screening is a series of tests to assess your risk for chronic health conditions that will provide you the opportunity to better manage your health. These tests are administered by a venipuncture blood draw.

When will biometric screenings be offered?

Biometric screening appointments will be offered starting Thursday, June 20.  Here are the screening options:

  • Attend your on-site screening from Thursday, July 18 through Thursday, October 31, 2019.
  • Visit a participating LabCorp location between Thursday, July 18 and Thursday, October 31, 2019.
Do I need to schedule an appointment?

Yes! We encourage all participants to schedule an appointment for their biometric screening. Details to schedule an appointment can be found in the program guide or listed below.

How long does the screening take?

You should expect the appointment to last for approximately 15 minutes.

Do I need to fast?

Yes. You are encouraged to fast for the most accurate screening results. Fast 10-12 hours prior to your scheduled screening time and drink plenty of water. Take nothing by mouth except water and medications. If you are diabetic or hypoglycemic, please follow your doctor’s instructions regarding fasting.

Who is conducting the biometric screenings?

The screenings are conducted by Wellness Corporate Solutions, a partner of Bravo Wellness. Both are reputable national organizations.

Are the test results confidential?

Absolutely. Only you, Bravo and Wellness Corporate Solutions have access to the results. Personal Health Information (PHI) is not shared with Hackensack Meridian Health. Hackensack Meridian Health strictly adheres to the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and Protected Health Information (PHI) protocols.

How do I register for the on-site biometric screenings?

Schedule an on-site screening beginning on Thursday, June 20. Follow the step-by-step instructions at http://www.bravowell.com/hmh to schedule a screening appointment at your work location. Screening dates for each location will be posted on your portal.

What if I cannot attend the on-site biometrics?

If you are unable to schedule an on-site appointment, you may have your biometric screening completed off-site at a participating LabCorp. Download a LabCorp voucher from your Bravo portal and schedule an appointment at a LabCorp location near you. Off-site biometrics will be offered starting Thursday, June 20 for the 2019 Wellbeing Initiative.

Do I need to submit my results if I complete the biometric screening at a LabCorp?

No, there is no need to submit your results to Bravo.

IMPORTANT - Any additional testing requests outside of the pre-approved panel will be at the expense of the participant.

What should I do with my biometric screening results?

Knowing your numbers is the key to your health. The data from your biometric screening provides you with valuable information about your health status. You may wish to discuss your results with your primary care physician.

When will I be able to complete my Health Risk Assessment (HRA)?

Beginning Thursday, June 20 visit http://www.bravowell.com/hmh and follow the step by step instructions to create an account.

How do I complete the HRA?

You are required to complete the HRA on Bravo’s website by visiting www.bravowell.com/hmh. Once you create an account and log in, complete the following two steps:

  • Step 1.   Register
  • Step 2.   HRA completion

Once completed, you will receive two reports: a personal report that describes the impact of each risk factor on your health including a wellness score, and a physician summary report that can be easily printed and shared with your provider to initiate a conversation. These reports will be available online for you to download/access. You will receive an email notification stating that your reports have been posted. No paper copies will be sent.

How do I complete an annual physical?

Schedule an annual physical with a participating provider, (HMH Partner or Horizon network provider). If you have previously completed your annual physical appointment after November 1, 2018, with an in network HMH Partner or Horizon provider, you are set! Your information will be provided by Horizon. This annual physical visit is covered at 100% when you utilize an in-network provider.

IMPORTANT - Remind your provider that the appointment should be coded as an “Annual Physical” so your insurance will cover it at 100%.

What if I had blood work done earlier this year with my doctor, can I earn credit for the biometric screening?

No, biometric screening results cannot be submitted by your doctor.

How do I check my completion status of the three steps?

Bravo will send email based notifications to team members and spouses with updates for completion status. Bravo will receive Annual Physical completion detail from Horizon on a bi-weekly basis. If you have already completed your Annual Physical, you will see this appear on your dashboard once received by Bravo.

How do I check my spouse/domestic partner/civil union completion status?

Each team member, spouse/domestic partner/civil union participant should create their own online accounts at www.bravowell.com/hmh. They can log in at any time to review their completion status.

What if I think my or my spouse/domestic partner/civil union completion status is incorrect on www.bravowell.com/hmh?

If you believe your completion status is incorrect, please contact the Bravo Wellness Call Center at 1-833-786-9492 or support@bravowell.com.

What is the deadline to complete the three steps?

In order to receive the incentive of $250, all three steps must be completed by October 31, 2019 and will be paid out shortly after our system registers that you have completed all three steps.

Can I complete the steps after the deadline?

No. You cannot complete the three steps after the initial deadline.  One lump sum incentive will be paid out to participating team members and spouses who are enrolled in the medical plan and complete the required steps.

How and when will I receive my incentive?

If all three steps are completed by the requested deadlines and confirmation has been received from the vendors, your incentive will be paid out shortly thereafter our system registers that you have completed all three steps.
